Supporting older Territorians
The Out of Hospital Care program provides unique 24/7 person-centred support and accommodation for older individuals who are preparing for transition from hospital into a longer-term residential aged care facility or appropriate home.
The program supports participants in shared accommodation, with 2 purpose-built villas on-site at Harry’s Place, and accessible homes in Darwin’s Northern Suburbs.
Reablement and reconditioning
The program delivers a holistic multi-disciplinary approach to support participant’s reablement and reconditioning goals. Participants have regular input from dedicated Carpentaria Occupational Therapists, Physiotherapists, and Allied Health Assistants to improve their quality of life and independence.

Our Out of Hospital Care team
The Out of Hospital Care program is led by Director of Aged Care Services and Registered Nurse, Claire Olssen, who has extensive clinical leadership experience in acute and subacute hospital care, Aged Care, and Disability Services. Claire is passionate about providing the highest quality services and supports to patients and participants and works collaboratively across care teams, internal and external stakeholders and family to support individual outcomes.
The program is supported by a Program Manager (Registered Nurse), Lifestyles Coordinator, 3 dedicated Independent Living Coordinators, an Allied Health Assistant, Occupational Therapist, Physiotherapist, and a team of experienced Independent Living Coaches (Support Workers).
