The Out of Hospital Care program provides unique 24/7 person-centred support and accommodation for older individuals who are preparing for transition into a longer-term residential aged care facility or appropriate home. The program supports participants in shared accommodation, with 2 purpose-built villas on-site at Harry’s Place, and modern, accessible homes in Darwin’s Northern Suburbs.
