Kim* is currently in year 12 and preparing for life after school. She is on the autism spectrum, is non-verbal and has sensory sensitivities and epilepsy. Kim’s guardian sought services that would support a gradual transition during her final year of school. 

Kim needs a considerable amount of time to transition into a new and comfortable routine. Following the ‘little and often’ Active Support principle, the Pathways Program team supported a gradual transition over a 12 month period, starting with regular short sessions at school before slowly introducing the program spaces and activities. 

Kim now attends the Pathways Program for 2 half days and 1 afternoon each week. In term 4, sessions will increase to 3 half days. Kim is excited to commence her full-time weekly program once school finishes.
