Upon transition from Palmerston Regional Hospital, Margaret and Harry* quickly settled into the Ready for Home program’s homelike care setting. Both live with dementia and required full time multi-disciplinary support prior to securing a longer-term residential care placement.

During their time in the program, they enjoyed activities including shared meals, walks and social community interaction, which fostered companionship and reduced feelings of isolation. Margaret and Harry’s developing friendship not only enhanced their emotional wellbeing, but encouraged participation in daily routines, resulting in improved mood, increased confidence and stronger engagement with their support team.

Margaret and Harry have now transitioned into the same residential aged care facility, with their existing friendship providing a sense of security which significantly minimised the stress often associated with such a transition. They have settled well into their new home and can often be found enjoying time together over a cup of tea in the garden.
