Tommaso* was admitted to hospital with dementia and multiple co-morbidities associated with age-related frailty. Once his medical requirements were resolved, and rather than staying in a clinical setting, he transitioned to the homely environment of the Ready for Home program.
This transition allowed Tommaso to experience a better quality of life, surrounded by the things that matter most to him. He was able to spend time with his family, share stories, and enjoy the simple pleasures that brought him comfort. One of the most meaningful aspects for Tommaso, an Italian-born Territorian, was the ability to enjoy home-cooked meals prepared with love – food that connected him to his culture, memories and family traditions.
The holistic and compassionate care provided through the program supported Tommaso to spend his final months living with dignity, maintaining vital connection with his loved ones, and experiencing joy and familiarity, despite his frailty.
