What is the NDIS?

The National Disability Insurance Scheme (NDIS) is a new and flexible way to provide long term individualised support to people with a disability.

The NDIS provides individuals with choice and control over the reasonable and necessary supports they require to live an ordinary life and achieve their goals.

It is a scheme which provides certainty that people with a disability will receive the care and support they need over their lifetime.

Am I eligible for the NDIS?

Participants need to be eligible to access the NDIS.

The criteria to be eligible include:

  • Age requirements
  • Residency requirements
  • Disability requirements or Early Intervention requirements

How Carpentaria can help you during your NDIS journey

PHASE 1: Engaging with the NDIS

The NDIS advises that these steps will support participants to:

  • Clearly communicate the NDIS’ role within the broader system of supports and services, including the role of the NDIS in providing to support to people with permanent and significant disability
  • Support people with disability to access the right supports for their disability and circumstance, including applying for access to NDIS-funded support if appropriate
  • Ensure people with a permanent and significant disability are referred to the access decision and receive a timely decision on access

STEP 1

Learn about the NDIS and how it fits in the broader system of supports

  • Contact our NDIS team to discuss the NDIS process and the services we provide
  • If you are an existing client receiving NT Government funding, your eligibility requirements may have already been met
  • If you are a new client, we can give you information to assist you in determining your eligibility

STEP 2

Decide to proceed to access

  • If you are an existing client, we can advise you whether the NDIS may already have some of your information to start the process
  • If you are new to the NDIS, we can provide you with information to help you through the NDIS pathway

STEP 3

Receive access decision

  • Under the NDIS, you can choose who your provider will be
  • When the NDIS advises you of access approval, Carpentaria can assist to identify suitable services and provide information regarding services fees and service delivery

PHASE 2: Planning to achieve goals and outcomes

The NDIS advises that these steps will:

  • Empower participants to set goals to achieve outcomes
  • Develop a plan to achieve outcomes which includes government, informal, community and funded supports to facilitate greater social and economic participation
  • Approve plans with reasonable and necessary funded supports

STEP 4

Understand current supports and prepare for planning

  • Talk to us about the services we offer and how we can contribute to the planning process
  • We can support you in preparing for your planning meeting

STEP 5

Create a plan to achieve goals and outcomes

  • As you work with the NDIA to create a plan, let us know if there is any specific information you require
  • We can also attend planning meetings with you and the NDIA representative

STEP 6

Receive approved plan

  • After the NDIA has notified you of your plan outcome, we can work with you to confirm the service delivery, set up a service agreement and provide you with a schedule of the supports we will provide

PHASE 3: Achieving goals and outcomes

The NDIS advises that these steps are designed to:

  • Activate plans and ensure that participants are connected to providers of supports which meet their needs
  • Ensure that participants are able to exercise choice and control in using their plans to achieve outcomes and monitor for any emerging issues
  • Review plans when appropriate, measuring outcomes achieved and setting goals for a new plan or exiting the Scheme as goals are achieved

STEP 7

Activate and implement plan

  • We will ensure that our service meets your expectations
  • We will also ensure that the service is aligned with the goals you want to achieve

 

 

 

STEP 8

Use plan to achieve outcomes

  • Our team will work with you to enable you to meet the goals in your plan
  • Our aim is to provide a service that delivers what you want to do and when you want to do it

STEP 9

Get support using plan

  • As you progress through your plan period, we can provide support and advice about any changes you may require to your services or overall plan content

STEP 10

Review outcomes and progress

 

  • When your plan is due for review (usually after 12 months), we can assist you with information to take to your planning meeting
  • We will also work with you to help identify and / or amend the goals for your next plan
